Where you ever banned from any kind of money earning program? Or perhaps rejected from the beginning? If yes, why did it happen? Was it really your fault? Did you receive an explanation, or you simply tried to access your account one day and had a surprise?

Ever rejected from money earning programs?

I was never banned. I always read the terms and conditions so i won't have surprises.
However, i was rejected a couple of times. Once i know i was rejected because my site R was not high enough for the program to accept it. The second time ... i have absolutely no idea why they did not accept me. I looked at the terms of service and i had all the needs met, but still the rejected me.
Oh well... tough luck i guess.

I also make sure I read all the terms carefully since I am an international user and some don't apply to me. Payment systems are also a reason for me to be careful since I might work and not get paid (now we have paypal too, so it's really getting easy).
it's important to read all the fineprint so there are no problems. There are so many ways one can earn online that one can always find a suitable system. Just read carefully, go with the plan and respect the rules.

Knock on wood nothing like this happened yet, but I am not into too many programs. I really try to be careful and not break the TOS and hope it's all gonna be OK. In 2008 I intend getting more into the money making business and this would also mean I'll have more program to join and more TOS reading to make. Let's hope it's all fine

None for me. It seems like most of the sites only go after the serious scammers, since it probably costs more than they lose to go after the small-timers...
