Dear eLab Panelist:
Greetings! I am writing you because you opted-in to receive periodic emails from eLab regarding activities and events.
Would you like to receive periodic emails (just a few times a year) from academics at other universities regarding participation in their research studies about online behavior? These studies also come with the chance to win cash incentives, just like eLab studies!
Please click the following link if you would like the chance to win cash incentives in other academic studies:

I loved the lab guy on their site; kinda reminded me of Beaker from the muppets.
The 'lab' part of the name makes them sound so clinical. Hope they don't ask for a "sample" if ya know what I mean! (joke, hahaha)

Dear Panelist,
Congratulations! You’ve been randomly selected as a winner for a participation award of $100 for your response in an eLab study we conducted back in October about a laptop or tablet PC evaluation. Please read and follow the following instructions carefully in order to receive your payment.
If you are a U.S. citizen or resident alien:
You must complete the attached IRS W-9 form (PDF format) and mail it to the address indicated on the form. Your form must be postmarked no later than two weeks from receipt of this notice for you to remain eligible. To protect your privacy, we ask that you do not send your completed W-9 via email reply.
If you are NOT a U.S. citizen or resident alien:
1) You must complete the attached "Statement of Services" form (PDF format) and mail or fax it to the address listed below. Make sure it is clearly labeled "ATTN: eLab". This form must be faxed or postmarked no later than two weeks from receipt of this notice for you to remain eligible.
2) You must reply via email with your full legal name, mailing address, and your citizenship status (including which country) within two weeks of receipt of this notice. U.S. citizenship is not required for payment, but this is information we must provide the IRS when reporting your earnings.
Why is a W-9 form required for U.S. citizens and resident aliens?
According to university policy, all payments to Individuals or Partnerships who perform a service to the University such as Caterers, Accountants, Attorneys, Photographers, Banks, Speakers, Honoraria, Subject Participants, etc. require an accompanying W-9, regardless of amount. These check requests must include full name, Social Security or Employer Identification Number, signature and a complete mailing address for federal reporting purposes. This information is not stored by eLab, but is passed on to the IRS in accordance with federal law and university regulations in order to ensure that total university distributions to individuals do not exceed the federal tax threshold of $600.
For more information on this policy, you may contact Sharon Stanley at shari.stanley@**************.
You can also refer to the IRS 1099 Miscellaneous under the Section When to Report. This form is available at www.irs.gov.
We'll send your check for $100 once we have this information. You should receive your check within about 6-8 weeks of our receipt of the form.
Thanks for your support of eLab. If you have any questions, please call or email me.
